230 DEVELOPMENT AND IMPLEMENTATION OF A STROKE PASSPORT IN THE HYPER-ACUTE STROKE UNIT: SUPPORTING PEOPLE POST-STROKE

Abstract Background A Multi-Disciplinary Team (MDT) approach is instrumental to post-stroke management in the Hyper Acute Stroke Unit (HASU). There strong evidence highlighting importance of patients’ social needs (UK National Clinical Guideline for Stroke, 2016) and need a clear plan resource combining both health care. This outlined Strategy (2020-2025). care involves cohesive multi-disciplinary approach. In 2021, members MDT HASU identified service gap implemented unified tool. The aim this project was develop user-friendly “Stroke Passport” ensure patient central their own stroke journey. Methods international guidelines were reviewed benchmarking other acute services completed. Passport specialised include key stakeholders including Consultant, Nursing Staff. Key contributed development adjustments made after feedback. sample patients piloted passport (n=10). Education provided staff through information sessions. Results resulted that captured following areas:(i)Causes (ii) Risk Factors (iii) (iv) Rehabilitation Goals (v) Useful Contacts (vi) Medication List (vii). Qualitative feedback from pilot study included: consistent awareness into type, rehabilitation goals discharge plan. successfully meet organisational demands while patient’s voice now embedded standard practice HASU. Conclusion implementation ensured inclusion goal setting planning. supports an integrated allows centred admission safe discharge.
